Thanks for the detailed post. I take it that Milnacipran (available in Europe only I believe) would also be a possible substitute although with the same caveat about NE stressing the adrenals.

What is the difference in activity between Milnacipran and Effexor?

I guess I should assume that boosting Cortisol levels with hydrocortisone to compensate for the adrenal fatigue is not a real smart option?
