My first guess would be as mentioned above would be a thyroid problem. Your TSH is high and your total
T4 is also low. Get the other tests JanSz listed @ Quest. (Total T3, Total
T4, Free T4, Free T3, TSH) Then you will know for sure. I can tell you this...my TSH and total T4 were almost exactly what yours are before I was diagnosed with hypothyroidism. After looking at Free T3 & Free T4 it turns out I was only producing a little more half the thyroid hormone my body actually needs. I also can relate to those symptoms big time...Even with my total T in the mid 600's I felt almost ZERO libido. Nothing will kill you sex life quicker than hypothyroidism! I am getting better slowly but surely now that I am being treated for this.
