Question Is TRT needed for low free T?

I had some basic labs done recently and noticed that total and free T were included. The Total was something like 385, with a listed reference range of 175-775. The free T was only about 4, with a reference range of 7-21.

Are these numbers bad enough that I should start thinking about TRT?

Your free T is an indicator of how much T is truly avaliable for your body to use. That just goes to show you how important it is to test for free T, or even better: bioavaliable T. Sadly many doctors would look at that 385 Total T and tell you that you were fine. I'm not quite as bad off as you and I went to see Dr. John.
If you don't have the means for a trip to Mich., I'd be looking for a good urologist. Your other option is looking for a anti-aging doctor in your area. A4M has a doctor search feature.

Test your
LH
FSH
Prolactin
SHBG
Albumin

If results indicate that you are secondary (low LH, low FSH) you may be better of using HCG in an effort to increase your overall testosterone.
You would follow (approximately) rules people use when trying to get pregnant.
Approximately HCG in doses (2000 -6000)/week.

If your SHBG is ether wery low or wery high, do further nvestigations, you may have other health issues.
Check your temperature when you are sitting calm for about one hour.
If it is much lower than 36.6C or 97.9F
check your thyroid
in that case it my be worth to look at thyroid and adrenals first, then testosterone.
