Default Bloodwork - please interperate

I am currently on no medication. My endo is an idiot and I need some real answers about these figures. Please help. I went to a different lab this time and some of their ranges aren't very good. I am 24 years old, not overweight, go to gym etc. I have empty sella syndrome which is basically where my pituitary is a bit squashed.


Cortisol a.m 585 nmol/L (160 - 650)
FSH <2 u/l ( <10 )
LH 3 u/l ( >9 )

IGF-1 24 nmol/L ( 24 - 102)

Prolactin 311 nMIU/L ( <500 )

Testosterone L 9.8 nmol/L ( 11 - 40 )

Free T4 11.1 pmol/L (9.0 - 19.0)
Free T3 4.5 pmol/L (2.6 - 6.0)
TSH 2.2 mU/L (0.3 - 5.0)


The only thing he told me was that my testosterone was a bit low which we already friggin knew. Can you guys please comment on my IGF-1 level and thyroids and cortisol? In past tests, my cortisol has always been pretty high and my IGF-1 has previously been tested at the exact bottom of range.

Your thyroid looks hypo, high TSH, T4 very low, Free T3 also quite low (under 50% of the range). Others would be better to comment on the treatment here, but I think you may benefit from amour.

Testosterone is definately low, with low LH, FSH showing secondary hypogonadism (pituitary not working properly). Did you get estradiol tested ?

Adrenals is hard to tell without more tests, as morning cortisol is near top of range which could be ok. Did you get DHEA-s, pregesterone, pregnenolone, aldosterone ? Would be good to do a 4xcortisol & 2x DHEA saliva test to see where your adrenals are.

I've had DHEA done in the past and it was right in the middle.

Cortisol has always been in the high end or over the top in all tests i've had for it.

My estrodiol is usually about 125 pmol/L (40 - 250)

I should also add that I have absolutely no confidence in these deadbeat endo's who empty my wallet and offer no knowledge or assistance. So I need all the help here I can get on whether or not I should try Armour, and what dose to start at.

Are there any risks involved in trying Armour at a low dose to see how I feel?

curious to see what your free cortisol level is. My freind had elevated cortisol on serum but low free levels this may explain why your t3 is higher then your t4 level. My mom had same thing and it was because of her coming off predisone so cortisol is deficnitely a factor as well a ferritin as well which hers was low ..

You would benefit from long blood test, similar to mine, attachment to post #1 here:

Done at Quest Diagnostics, blood drawn at Quest.

With the test that you presented;
Try to boost you natural T, use Novarel HCG 250iu/EOD
retest after 2 months if it is not enough use T supplementation.
Test for
(Free BioAvailable and Total Testosterone)
Either FreeT or BAT should be on top of the range leave the other be.
That would apply if you supplement on T, if on HCG only, you may not reach thoes levels but the should be quite high to be acceptable.
If you have to, keep HCG and add T. Most likely T shots,
Depo®-Testosterone---200 mg/mL
There are different ways to use this combination, presently, for effectiveness and convenience in daily life I favor
(T + HCG) both shots SubQ around navel, every other day, have shots free the next day.
100mg T weekly dose, use 28 or 29ga 1/2cc insulin syringe for T and same or thinner for HCG
100mg/week=0.5mL/week
E2D dose = 0.5/7*2=0.143mL=0.143cc=14marks on insuline syringe, '=0.14/2*7*200=98

Test after 6 weeks, adjust dose as need.

Make sure you get good estrogen testing, specially when your will be high on T.

Your FreeT3 is low, but it is not that bad overall, so not first priority.
Use Armour Thyroid to raise it to top of the range or close to it at about 20%
Use Corteff, 2-3 times /day max 20mg to support Adrenals.
Go by how you feel for both Cortef and Armour.

Free (T3) --4.5 pmol/L (2.6-6.0)

5.320 <--FT3 start of upper 20%
5.150 <--FT3 start of upper 25%
4.867 <--FT3 start of upper 33%
4.300 <--FT3 start of upper 50%
44.12 <--your current 4.5 puts you %% from the top =100-(4.5-2.6)/(6.0-2.6)*100
