Default Clomid for TRT?

Ths study was completed last month. i found it quite interesting. These men were treat with clomid only for their hypogonadism and were folled up on over a period of 6 months to over 3 years. it seems that clomid may indeed be a possible treatment for hypogonadism if you can tolerate the side effects. The men were given 50mg EOD.

ANyone have any thoughts on this?

I am thinking it would be a superior form of treatment due to keeping the HPTA intact. Also on average mens sexual function on Clomid was somewhat better than it was before treatment.

ABSTRACT
Introduction. The efficacy of oral clomiphene citrate (CC) in the treatment of male hypogonadism and male infertility (MI) with low serum testosterone and normal gonadotropin levels has been reported.

Aim. The aim of this article is to evaluate CC and testosterone gel replacement therapy (TGRT) with regard to biochemical and clinical efficacy and cost.

Main Outcome Measures. The main outcome measures were change in serum testosterone with CC and TGRT therapy, and change in the androgen deficiency in aging male (ADAM) questionnaire scores with CC therapy.

Methods. Men receiving CC or TGRT with either Androgel® 1% or Testim® 1% for hypogonadism (defined as testosterone < 300 ng/mL) or MI were included. Serum values were collected 1–2 months after treatment initiation and semi-annually thereafter. Retrospective data collection was performed via chart review. Subjective follow up of patients receiving CC was performed via telephone interview using the ADAM questionnaire.

Results. A hundred and four men (65 CC and 39 TGRT) were identified who began CC (50 mg every other day) or TGRT (5 g). Average age (years) was 42(CC) vs. 57 (TGRT). Average follow up was 23 months (CC, range 8–40 months) vs. 46 months (TGRT, range 6–149 months). Average posttreatment testosterone was 573 ng/dL in the CC group and 553 ng/dL in the TGRT group (P value < 0.001). The monthly cost of Testim® 1% (5 gm daily) is $270, Androgel® 1% (5 gm daily) is $265, and CC (50 mg every other day) is $83. Among CC patients, the average pretreatment ADAM score was 4.9 vs. 2.1 at follow up (P < 0.05). Average pretreatment ADAM sexual function domain score was 0.76 vs. 0.23 at follow up (P < 0.05). There were no adverse events reported.

Conclusion. CC represents a treatment option for men with hypogonadism, demonstrating biochemical and clinical efficacy with few side effects and lower cost as compared with TGRT. My doc uses lower-dose Clomid as a treatment. Clomid worked really well for me, but I'd be uncomfortable running in consistently or as a longterm treatment because of the side effects. It can lead to clotting issues, which can be very severe. If you get a bad embolism, you may have no second chances...

On a clomid only treatment I could not imagine there would be e2 issues like there are on traditional TRT. I may be wrong. But if the HPTA is working it would seem it would regulate itself.

Good Thread I have one guy on Clomid for low T at the H2 forum he has been on this for yrs and doing well. But others that read his posts tried it and could not take the sides most of the sides were EYE problems.
You can go to the H2 forum and join and do a search find this guy and read his posts.

this study raises more questions than it answers. For instance, notice the difference in the ages of the subjects. The clomid group were on average a decade or more younger. The pretreatment testosterone levels are not given. While there may be value to clomid as a stand alone testosterone enhancer, we couldn't use this study to warrant it. A few more carefully controlled studies could clarify the matter. Another side effect sometimes experienced with clomid is depression. Back in the day, I was working w. a doc who was using prohormones as testosterone boosters. These were cycled off and on. After several months, clomid was used. Once the clomid was terminated, levels were checked and TRT was started. The post clomid values were approximately what they were before I started treatment. I was in my late 50's. So, at least as per this sited study, I was not a good canditate for clomid as a stand alone intervention. So age alone may be the most salient feature of the sited study and the positive results. If you are in your 40's and suffering some form of hypogonadism, a clomid trial may be worth the effort---hopefully you would not be in that group experiencing some of the sides.

WHat were your values during clomid treatment? Was clomid at least able to elevate your LH ans FSH numbers?

OK. So I took my first 25 mg clomid yesterday and well I feel worse actually No morning oaks I had some the morning before I took it, basically low e2 symptoms. Anybody can give me any input. I feel worse actuallty. I am guessing it takes a few days for it to start making my pituitary shell out natty test. of I mean the signal to create to natty test. I am kinda disheartened but I am going to give this a chance. Can anyone give some input?

BTYW. last T shot was 17 days ago and last hcg shot was 7 days ago,. Up until I took the clomid I was not doing that bad actually.

ive been on 50mgs eod for over a month and i still feel like crap. but im having morning wood spontanous erections and even had two nocturnal emissions.but im so fatigued.and i dont feel any stronger my workouts suck and feel very down..but i think its working in the test department its just the estrogen issues im guessing
