Default Input on some labs

A client of mine wanted to have some input on his recent labs. He's a 36 yo male that's been feeling extreme, chronic lethargy and problems losing weight. His diet and exercise routine is great. He tried to get the entire list of labs that SWALE uses, but this is what he could get.

Glucose 102 (65-99)
Urea 18 (7-25)
Bun/Creatinine Ratio 15 (6-25)
Sodium 141 (135-146)
Potassium 4.4 (3..5-5.3)
Protein, Total 7.2 (6.0-8.3)
Albumin 4.8 (3.7-5.1)
Globulin 2.4 (2.2-4.2)
Albumin /Gobulin ratio 2.0 (0.8-2.0)
Bilirubin Total 0.5 (0.2-1.5)
Alkaline Phosphatase 85 (20-125)
AST 21 (3-50)
ALT 45 (3-60)

Trigcycerides 91 (<150)
Cholesterol, total 170 (<200)
HDL 56 (>40)
LDL 96 (<130)

Y-4, FREE 1.4 (0.8-1.8)

Cortisol, A.M 9.5 (4.0-22.0)

His Cortisol is low, if he could test DHEA Sulfate, then it would give a better picture of his adrenals. His Glucose is high, not sure what that means. For proper thyroid evaluation he would need TSH, Free T3 and Free T4.

This is just my layman’s opinion so take it for what it’s worth. First thing I'd look at is diabetes myself. If that reference range is right his Glucose is way too high even if that wasn't a fasting glucose. Four times the upper limit of normal is incredibly high and could easily account for his symptoms.

The testosterone looks okay as his Free T is in the upper half of normal and I seriously doubt he’d have any of the symptoms you describe at that level.

He needs to get more testing for diabetes IMO. And diabetes is very serious, I'd imagine if a doc looked at that level he'd be in that same day for more tests.

I think you misread it. the upper limit is 99 and his level as 102, not four times the upper limit

It was recently edited. The upper limit was much lower, something like 40.

Okay, that's MUCH different. I had never seen a Glucose range that ended at 25 so I was wondering. So diabetes isn't going to be it. :-) All the other blood work looks okay to me. Sure, some things can be optimized but nothing I can see looks like it could cause the symptoms you describe. I’d also check Free T3 and TSH just to make sure it’s not thyroid. If you’re client has signs of low Cortisol production (look it up) you might want to test it. In my search for an answer to my symptoms I’ve been through all these tests, the only one coming back abnormal was Testosterone...lucky me! :-( Too bad it was practically the last test I was going to try.

His GP is sending him to an endo for the blood glucose level. They are trying to rule out diabetes. I don't see why his blood glucose would be at such a high level while fasted really. He has no family history.

My initial thoughts were to get a complete thyroid panel and to further investigate the cortisol.
