Default Testosterone therapy and having problems

Hey Guy's,

I've appreciated the advice i've had on this board in the past and need your help. I've been on a compounded Test Cream of 300mg per CC and take 1 cc every day. I'm also taking 1mg of Arimidex on Wed and Sunday. I take 50mg of Pregnenalone and 75mg of DHEA a day. I also take 6 grains of armour thyroid a day.

Here's my problem. I'm still having problems maintaining an erection. I've also has some irritation of my prostate (Feel like I need to urinate all the time) I am taking Saw Palmetto. I'm 38 years old. I've been on treatment for about 6 months. My recent PSA came back normal at 1.0 (range .5 to 4.o) Here are my recent labs and wonder what you think:

DHEA 186 (range 110-370)
Estrogens, Total, Serum 251pg/ml
Testosterone, Total 367ng/ml
%Free Testosterone 1.88%
Testosterone, Free 69 pg/ml
Prengenalone 20 ng/ml
Leutinizing Hormone <.2
FSH <1.0 ng/ml
PSA 1.0ng/ml (Range .5-4.0)

Why are my estrogens so high why I'm taking arimidex? Should I increase the dose of Test? Could my prostate be irritated by a build up of DHT? Any help would be appreciated. My doctor has been great about several things but I also need your advice.

Symptoms sound like prostatitis or possibly a urinary tract infection. If a round of antibiotic doesn't turn it around it's possibly a yeast infection. Also looks like your aromatase enzymes are high, maybe from all the androgen precursors you are taking and the arimidex dose isn't high enough.

Thanks for your input. I thought the androgen precursors could be a problem. I asked my doctor but he say's I need them. With my PSA being normal do you believe my prostate is OK otherwise? How should I dose the Arimidex and should I DC the androgen precursors. Also should I have my doctor increase my Test cream dose? Thank you again for your input.

Does anyone have any additional Comments. I really need your help. How should I proceed? Should I stop the pregnenalone and DHEA? Should I tincrease my Arimidex dose. Should I Increase my test osterone? Any help would be apprecited.

It would help if you could include the ranges for all the tests that you have listed.

You're already on a high transdermal dose, yet your testosterone levels aren't that high - need the ranges to be sure. When twice daily dose of 200mg cream was mentioned, Dr. John (SWALE) had the following to say -

His comment was about a dose that is only 33% more than your dose, yet your testosterone levels are not that high. I would have to say that you are not absorbing the cream well. Maybe it is time for injections.

Thank you for your comment. My doctor didn't check my estradiol but did check my serum estrogens. They do seem High. I understand the estradiaol level and what it could do. What would be the benefit of checking DHT? Could that be the cause of my prostate issues. Your help is greatly appreciated.

The reason that I ask about your estradiol is that it is much more significant than total estrogens. So much so that Dr. John has this to say -

You have a problem when DHT is to low you can be 2 x's over the top of the range and not have problems.

I want to tell you how important you guy's are here to the board. Programer from what you sent it sounds like my high estrogens could be the problem. I will get my estradial checked. Do you guy's think the pregnenalone and DHEA are converting more to estrogen or do you belive it's my Test cream that's the problem. I've been on the injections before but had problems. (Mainly Estrogen) my doctorr didn't have me on Arimidex. I'm taking Arimidex 1mg 2 times a week. I'm going to try it 3 times a week and cut out the DHEA and Pregnenalone. Let me know what you think. Thank you!

About a year ago my total estrogen was 250. My Doctor perscribed me arimidex. I took
1 mg per day. The gel's are easy to aromatize. Go with the shots. I got my blood tested
and total estro droped below 70. Don't worry about sides. You should actually be
using your E-2 est levels for bench mark. There are three types of estro and E-2 is
the only one men should be concerned with. Mine is down to 15. I've backed down to
1 mg every other day. The DHEA and Prengenalone are things I have in my closet
that I think would be better off in the trash.

Do you have a thyroid problem.? If so just do what your Doc says. I have no clue as
how that interacts. Good Luck!
